New Course and Certificate Program Trains CHWs

South Mountain Community College offers an entry level class and certificate of completion designed to train individuals to become Community Health Workers.

Community Health Workers, also known as Health Advisors, Peer Health Promoters or Patient Navigators, work to connect individuals and families with the appropriate health and human services that they need, while providing outreach, education, referral and follow-up, case management, advocacy and home visiting services. 

The entry level class, Community Health Work (CHA104), focuses on all of these topics and more. The course also includes 30 hours of service learning experience in the field.

The class fulfills 3 credit hours of the requirements for the 16 credit hour Community Health Worker Certificate of Completion.  Click here to view courses required for the Certificate of Completion.

Spring 2012 Course Schedule

Community Health Work (CHA104, Class Nbr 24356) will meet once a week on Tuesday evenings,  January 24 through May 11, 2012, from 6:00 to 8:40 p.m. nightly.

The class will take place at the SMCC Main Campus, 7050 S. 24th Street, in Phoenix, just north of Baseline Road.

Tuition for the three-credit class is $228 plus a $15 registration fee for residents of Maricopa County.  For those living outside Maricopa County, tuition is $303 plus a $15 registration fee.
